Voting Eligibility Cover Sheet
Confirms a shareholder's eligibility to vote at the Annual Meeting. Submitted with proxy or voting materials so non-voting shares can be reissued with full voting rights once Alaska Native status is documented under 43 U.S.C. § 1602(b).

Stock Will (Testamentary Disposition of ANCSA Stock)
Direct your Emmonak Corporation shares under Alaska Statute 13.16.705(b) and ANCSA § 7(h)(2). Complete the wizard online for a personalized draft, or download the printable blank to fill out in person. In both cases you must sign before two adult witnesses and either notarize OR postmark before the document is legally effective.

ANCSA Stock Inheritance Affidavit
Filed by an heir or personal representative when an Emmonak shareholder passes away without a valid Stock Will, or when a Stock Will exists and must be confirmed against ANCSA succession rules. Executed under AS 13.16.705(a) with reference to 43 U.S.C. §§ 1606(h) and 1607(c).

ANCSA Inter Vivos Gift of Stock (Rev 0.1)
Transfer ownership of Emmonak Corporation shares while both giver and recipient are living, under ANCSA § 7(h)(1)(C)(iii). Both parties must be Alaska Native under 43 U.S.C. § 1602(b).

Shareholder-benefit and customer-finance applications route to the accounting office with the President & CEO on copy for the approval decision.
Emergency Funds Request
A once-per-fiscal-year (calendar year) shareholder benefit of up to $500.00 for qualifying medical, death, disaster, or other emergencies affecting you or your immediate family. Decisions are made by the President & CEO within ten business days of complete documentation.

Customer Credit Application — Individual
Open a Net 30 personal account with Emmonak Corporation for purchases at the Tank Farm, Company Store, or Asset Rentals. The finance charge on past-due balances is 1.5% per month (18.0% APR). Subject to credit approval by the President & CEO; decisions are returned in writing within ten business days.

Customer Credit Application — Business
Open a Net 30 commercial account for businesses purchasing from any Emmonak Corporation business unit. The finance charge on past-due balances is 2.0% per month (24.0% APR). A purchase-money security interest under the Alaska UCC applies. May require a personal guaranty for newer entities.
