Submit this short intake and the Records Custodian will contact you via secure channel to collect a CDIB, tribal enrollment card, or equivalent documentation before your shares are reissued with full voting rights.

After you submit, the Records Custodian will contact you through a secure channel to verify identity and collect any additional information needed.
